NASC Support Services
NASC is short for the Needs Assessment & Service Coordination Service. NASC services are the organisations contracted by Lakes DHB to work alongside people in the community who require support to remain living in the community or transition to residential care. When the time is right, you can contact your local NASC agency or be referred via a Dementia Lakes Support Coordinator, your GP or other health professionals. A case manager will then get in touch with you and your family to determine what services are needed such as assistance with household tasks or personal care needs (showering, dressing or supervising daily medication).
